Apabila Peserta Didik pindah sekolah, maka laporan Penilaian Hasil Belajar Peserta Didik ini dibawa oleh yang bersangkutan untuk digunakan di sekolah baru dengan meninggalkan arsip/copy di sekolah lama.
Apabila Laporan Penilaian Hasil Belajar Peserta Didik yang bersangkutan hilang, dapat diganti dengan Laporan Penilaian Hasil Belajar Peserta Didik Pengganti yang nilai-nilainya diambil dari Buku Induk Sekolah Asal Peserta Didik dan disahkan oleh Kepala Sekolah Asal.

Laporan Penilaian Hasil Belajar ini harus dilengkapi dengan pas foto Peserta Didik ukuran 3 cm x 4 cm, pengisiannya dilakukan oleh Guru Kelas/Walikelas, berdasarkan data penilaian hasil belajar dari guru mata pelajaran terkait.

Pengisian Laporan Hasil Belajar Peserta Didik ini mengacu pada Panduan Penyusunan Laporan Hasil Belajar Peserta Didik Sekolah Menengah Atas.
