Efek toksik bilirubin ialah neurotoksik dan kerusakan sel secara umum. Bilirubin dapat masuk ke jaringan otak. Ensefalopati bilirubin adalah terdapatnya tanda-tanda klinis akibat deposit bilirubin dalam sel otak. Kelainan ini dapat terjadi dalam bentuk akut atau kronik. Bentuk akut terdiri atas 3 tahap; tahap 1 (1-2 hari pertama): refleks isap lemah, hipotonia, kejang; tahap 2 (pertengahan minggu pertama): tangis melengking, hipertonia, epistotonus; tahap 3 (setelah minggu pertama): hipertoni. Bentuk kronik: pada tahun pertama: hipotoni, motorik terlambat. Sedang setelah tahun pertama didapati gangguan gerakan, kehilangan pendengaran sensorial.
