Ikan yang digunakan untuk pembuatan pempek sejatinya adalah ikan belida (Notopterus chitala). Namun, karena semakin langkanya ikan tersebut maka ikan yang digunakan untuk pembuatan pempek pun menjadi lebih bervariasi, seperti jenis ikan sungai yang lain, yakni ikan gabus (Channa striata), ikan Putak (Notopterus notopterus), ikan toman (Channa micropeltes), ikan sepat siam (Trichogaster pectoralis), dan ikan bujuk (Channa lucius). Beberapa jenis ikan laut, seperti ikan tenggiri (Cybium commersoni), ikan parang-parang (Chirocentrus dorab), dan ikan kakap merah (Lutjanus argentimaculatus) juga dijadikan bahan pembuatan pempek. Pada prinsipnya, semua ikan air tawar dan laut bisa dipakai untuk bahan membuat pempek, tetapi ikan laut lebih amis baunya. Namun, pada umumnya untuk mendapatkan rasa pempek Palembang yang enak hanya akan menggunakan tiga jenis ikan, yakni Belida, Gabus, dan Tenggiri.Namun memakan pempek secara berlebihan juga menimbulkan efek samping yaitu alergi pada kulit akibat kekebalan tubuh,jika dikonsumsi oleh ibu hamil makan akan mengalami gangguan kecerdasan,cuka kuah pempek dapat menyebabkan gigi keropos terlalu banyak mengosumsi pempek juga dapat mengganggu kesehatan manusia.
